Silverchain is seeking a Care Partner to support older Australians in the Central Coast region.
The role blends community-based client visits with the flexibility to work autonomously from home, managing a caseload and delivering personalised care plans.
The position is full time for 6 months, with standard hours 8:30am–5:00pm, Monday to Friday, and opportunities to collaborate with clinical, scheduling and service teams.
Police check and vaccination status may be required.
