Care planning with real clinical backup

Most care partners carry a big caseload and chase allied health from outside the building. At Avanza Care, you’ll work right alongside our own allied health team, and alongside the clinical lead you can go to on complex clients.

Our focus is helping clients get stronger, stay safe and keep living the life they want at home – we are wellness and reablement focused first. We’re a young Support at Home service, which means you won’t be inheriting years of bad habits. You’ll help us build it from the ground up.

Avanza Care is taking a fresh approach by embracing the philosophy of Support at Home which is to embed wellness and reablement into care planning and empower our clients to invest in themselves by regaining/maintaining strength, balance, mobility and independence. We are looking for a care partner who will also embrace this fresh approach to care management.

What you’ll do

• Own a caseload of Support at Home clients, from onboarding through to review

• Build care plans, service agreements and budgets with clients and their families, so their funding is working hard for them

• Coordinate the services in each plan, delivered by our team and our partner providers, and adjust quickly when needs change

• Keep an eye on budgets and spending, and step in early when things start to drift

• Run regular reviews and reassessments, bringing in our allied health team when a client would benefit

• Be the go-to person for clients, families, GPs and community services

• Keep clear, accurate records that protect clients and stand up to audit





• Help our leadership team shape how the care model works in practice

About you

• Experience as a care partner, care coordinator or case manager in Home Care Packages or Support at Home

• Experience with AlayaCare an advantage

• Confident running your own caseload and making day-to-day calls

• Strong with care planning and budgets, and you know the funding rules well

• You build trust with clients and families quickly, including in the hard conversations

• Calm and organised when priorities compete

• Current driver licence and own reliable vehicle

• You also hold – Recent National Police Check Certificate, First Aid and CPR Certificates, and are able and willing to complete on the job training to upskill and remain up to date with best practice.

What you’ll get

• $95,000 – $105,000 FTE pro-rata + super

• Part time hours: 4 days per week, flexibility on start/finish times

• A mix of office time in Hyde Park, home visits, and where appropriate working from home

• Kilometre reimbursement for work related travel, work laptop

• PD allowance

• Direct access to in-house allied health and clinical leadership – you’re not on your own with complex clients
