About the Role
We are seeking an experienced and passionate Care Coordinator to join our Sydney team. In this role, you will coordinate care services, support clients throughout their care journey, and work closely with clients, families, Care Partners, and service providers to ensure quality outcomes.

Key Responsibilities

- Coordinate and manage client services to ensure high-quality care delivery.
- Develop, review, and maintain client care plans and service documentation.
- Act as a key point of contact for clients, families, and stakeholders.
- Monitor client wellbeing and respond proactively to changing care needs.
- Coordinate services and supports to meet client goals and preferences.




- Maintain accurate records and ensure compliance with organisational policies and funding requirements.
- Liaise with internal teams and external providers to ensure seamless service delivery.
- Contribute to continuous improvement initiatives and quality outcomes.

About You You are a compassionate and organised skilled with a strong commitment to delivering person-centred care. You have excellent communication skills, can manage competing priorities, and thrive in a fast-paced environment.
Essential Skills & Experience

- Previous experience in a Care Coordinator, Care Partner, Case Manager, Support Coordinator, or similar role.
- Experience within aged care, community care, disability, health, or human services.
- Strong understanding of person-centred care principles.
- Excellent organisational, communication, and stakeholder management skills.
- High level of computer literacy and experience using client management systems.
- Ability to work independently and collaboratively within a multidisciplinary team.

Desirable
- Qualification in Community Services, Aged Care, Nursing, Health, Social Work, or a related field.
- Understanding of Support at Home, Home Care Packages, CHSP, or community care programs.
- Current Driver's Licence and willingness to travel within the Sydney region.
