Bayside Health's Transition Care Program in Frankston, Victoria is seeking a Grade 2 Physiotherapist. You will deliver physiotherapy assessment and management across aged care streams, supporting protected transitions from hospital to home within a multidisciplinary team.

This fixed-term Full Time role offers 80 hours per fortnight until December 2027, with salary commensurate with VB1-VB4 levels plus super, located at Golf Links Road, Frankston.
