McArthur Aged Care is seeking an experienced and confident Registered Nurse to join a highly regarded, national not-for-profit aged care provider in Sunshine as a Clinical Care Partner.
This is a fantastic opportunity for an RN with community aged care experience who enjoys combining clinical care management with leadership, mentoring and supporting other team members.
The position is initially a locum assignment through McArthur, with the chance to transition into permanent employment should both parties feel it's a good fit.
About the Role
Working within the Support at Home program, you'll provide clinical oversight and care management for a caseload of approximately 20 clients across Support at Home classifications 1-8, with varying levels of clinical complexity.
You'll play an significant role in ensuring clients receive safe, high-quality, person-centred care while also providing clinical leadership, guidance and support to Care Partners and other team members.
Key responsibilities include
- Managing a caseload of approximately 20 community aged care clients with varying clinical needs.
- Conducting comprehensive clinical assessments and developing, implementing and reviewing individualised care plans.
- Monitoring changes in clients' health and coordinating appropriate clinical interventions.
- Providing clinical leadership, guidance and mentoring to Care Partners and other team members.
- Supporting colleagues in managing clients with complex or changing health needs.
- Working collaboratively with clients, families, support workers and healthcare professionals.
- Providing hands-on clinical nursing support in the community when required.
- Ensuring clinical care aligns with aged care quality standards and best-practice principles.
About You
We're looking for an experienced Registered Nurse who is confident working autonomously and enjoys sharing their clinical knowledge with others.
To be considered, you will need:
- Current AHPRA registration as a Registered Nurse - essential.
- Previous experience in community aged care - essential.
- Demonstrated clinical leadership experience, with the ability to guide and support other team members.
- Strong clinical assessment, care planning and case management skills.
- An understanding of Home Care Packages and/or the Support at Home program.
- Excellent communication, organisational and problem-solving skills.
- Confidence managing clients with complex clinical needs.
- Sound IT skills and experience using electronic care management systems.
- A current driver's licence and willingness to undertake community-based client visits.
Experience using AlayaCare would be highly regarded.
