About the role

My Disability Support Services is seeking a compassionate and dedicated Social Worker - Disability to join our team in Greater Sydney NSW. This is a full-time position (can be Contractor) where you will play a vital role in supporting individuals with disabilities and their families to navigate complex social, emotional and practical challenges. As a Social Worker specialising in disability support, you will be instrumental in improving the quality of life for our clients and helping them achieve greater independence and wellbeing.

What you'll be doing

- Conducting comprehensive assessments of clients' social, emotional, physical and practical needs to develop individualised support plans

- Providing direct counselling and psychosocial support to individuals with disabilities and their families

- Advocating on behalf of clients to access appropriate community resources, services and entitlements

- Coordinating care and support services across multiple providers to ensure holistic and integrated support

- Liaising with healthcare professionals, educators, employers and other stakeholders to facilitate coordinated care and support

- Supporting clients in developing coping strategies and building resilience to manage their disability and life circumstances

- Maintaining detailed, accurate and confidential client records in compliance with relevant legislation and organisational policies

- Participating in case conferences and multidisciplinary team meetings to review client progress and adjust support plans as needed

- Contributing to the development and evaluation of social work programmes and service improvements





- Providing crisis intervention and support when clients experience acute emotional or social difficulties

What we're looking for

- A recognised qualification in social work, including a Bachelor's degree or higher qualification (such as a Master's degree) in Social Work or a related discipline from an accredited institution

- Current registration with the Australian Association of Social Workers (AASW) or eligibility for professional registration

- Demonstrated experience working with individuals with disabilities, preferably in a support, counselling or case management capacity

- Strong knowledge of disability support services, relevant legislation (such as the National Disability Insurance Scheme), and the disability rights framework

- Excellent communication and interpersonal skills, with the ability to work sensitively and respectfully with people from diverse backgrounds

- Demonstrated ability to conduct thorough assessments, develop evidence-based intervention plans and evaluate outcomes

- Experience in crisis intervention and de-escalation techniques

- Strong organisational and time management skills, with the ability to manage multiple cases and competing priorities

- Proficiency in computer systems and electronic record keeping




- A commitment to social justice principles and a genuine passion for supporting people with disabilities to achieve their goals

- Understanding of trauma-informed practice and cultural competency in service delivery

- Current Working with Children Check (or willingness to obtain one) and relevant police clearance
