Dorset HealthCare University NHS Foundation Trust is seeking an Older Persons Community Support Worker to join the Community Mental Health Team for Older People in Wimborne. You will support older adults with dementia, depression, anxiety, and other mental health conditions in their homes or community settings.

You will work with a multidisciplinary team to deliver recovery-focused, person-centred care, visiting service users, monitoring wellbeing, and helping maintain independence.
