The Royal Melbourne Hospital’s Royal Park Campus is seeking a Case Manager for the Transition Care Program - Home Based. You will coordinate short-term, goal-directed care for older adults following hospital discharge, working with clients, families and health professionals to support independence and safe home living.

This fixed-term role (until January 2027) offers a 64-hour fortnight schedule across four days, with a team-oriented multidisciplinary team.
