Liverpool University Hospitals NHS Foundation Trust is seeking an experienced Complex Discharge Nurse to join the Complex Discharge Team. The role focuses on coordinating protected, timely and person-centred discharges for patients with complex health and social care needs, working with ward teams, community partners and local authorities.

The post holder will lead discharge planning across pathways, ensure EDDs are documented, and identify barriers to reduce delays while improving patient outcomes
