Cambridge University Hospitals NHS Foundation Trust is seeking an Advanced Clinical Pharmacist for the Virtual Ward to support protected, effective hospital-level care at patients’ homes using remote monitoring, telehealth, and face-to-face assessments.

You will work within a multidisciplinary team to assess patients, plan treatment, monitor progress, and coordinate discharge, helping to reduce admissions while delivering high-quality care in familiar surroundings.
