My Guardian is seeking a dedicated Care Coordinator to join our Sydney Olympic Park Head Office on a full time, on-site basis. You will coordinate staff rostering, build relationships with clients and families, and provide on-call support to ensure continuity of care.

You will work with clients, carers, and Case Managers to achieve care goals, assist with NDIS and aged care program coordination, and contribute to smooth office operations, maintaining records and stakeholder communications.
