Bolton Clarke is seeking a Care Partner to manage care delivery for Home at Home participants from our Dandenong South office. You will plan and oversee care services, coordinating with clinicians to tailor support to each participant's goals and needs.

The role requires Cert III in Individual Support or a Diploma-level qualification in Nursing, Community Services or Aged Care, and solid communication, empathy and cultural competence. A driver’s licence and access to a car are expected.
