Calvary Home Care - Darwin is seeking a Case Manager to oversee a caseload under CHSP/HCP funding. You will manage budgets, onboard recent clients and coordinate care to maintain client independence.

You bring leadership in case management, experience in aged care or disability sectors, and a willingness to obtain NDIS and police clearances. A full driver's licence and vehicle are preferred to support community visits.
