The Royal Melbourne Hospital invites applications for a Case Manager in the Transition Care Program – Home Based at the Parkville Campus. You will co-ordinate short-term, goal-focused care for older adults after hospital discharge, working with patients, families and health professionals to optimise independence and support protected home living.

You will conduct assessments, liaise with service providers for in-home services, and help clients access ongoing supports as they transition to long-term
