- Fixed Term Full Time, 80 hours per fortnight until December 2027

The Team & Role

Join Peninsula Health's Transition Care Program (TCP), a multidisciplinary service that supports older people at the interface between hospital, home and the community. As a Grade 2 Physiotherapist, you will play a key role in delivering high-quality physiotherapy assessment and management across a broad range of aged care clinical streams, helping to optimise patient outcomes and support protected transitions from hospital care. Working within a collaborative multidisciplinary team, you will contribute to person-centred care, goal setting,



rehabilitation and service improvement initiatives while supporting positive outcomes for older people across community and home-based settings throughout the Mornington Peninsula.

Skills & Experience Required

- Bachelor of Physiotherapy, Masters of Physiotherapy Practice.
- Current general registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- A minimum of 2 years professional experience in working with older adults.
- Demonstrated participation in Professional Development.
