Job Title: Community Worker

About the Role:

We are seeking passionate and experienced Community Worker in Wangaratta to support clients with complex behaviours.

Key Responsibilities:

- Engage with community members and groups to identify unmet needs and available resources in health, housing, employment, training and welfare.
- Assist in planning and delivering community initiatives to address service gaps and encourage social inclusion and community participation.
- Establish and maintain working relationships with community organisations, welfare agencies, government departments and local businesses to coordinate support and promote available services.
- Identify suitable support services, facilitate client referrals, assist with access arrangements and monitor referral outcomes.
- Track client progress, maintain accurate case notes and records, and prepare reports to evaluate and improve support plans.
- Assist with preparing funding applications,



resource requests and reports outlining community program activities and outcomes.
- Support participants with daily living tasks, community engagement, and personal care
- Accurately record shift notes and report any incidents or changes in behaviour

Essential Requirements:

- Minimum 6 months experience working with clients with behavioural needs
- NDIS Worker Screening Check
- Working with Children Check (WWCC) – NSW
- Valid First Aid and CPR Certificate
- Valid Australian Driver’s Licence and access to a reliable vehicle
- COVID-19 Vaccination Certificate (if required by client)
- Solid verbal and written communication skills
- Willingness to travel within Wangaratta region
