About the Role

As a Support Coordinator, you will assist NDIS participants to understand and implement their NDIS plans, connect with appropriate services, and work towards greater independence and improved quality of life.

Location: Point Cook, VIC
Work Arrangement: Primarily office-based, with work from home opportunities

Employment Type: Casual

Key Responsibilities
Provide support coordination to NDIS participants in line with their individual goals and needs.
Assist participants in understanding and implementing their NDIS plans.
Connect participants with suitable community, health, and disability support services.
Build and maintain positive relationships with participants, families, carers, and service providers.
Coordinate services and monitor participant progress.




Maintain accurate case notes, reports, and documentation.
Advocate for participants to promote choice, control, and independence.
Ensure all services are delivered in accordance with relevant NDIS requirements. About You

We are looking for someone who:
Has experience in NDIS Support Coordination .
Has robust communication, organisational, and time-management skills.
Is passionate about supporting people with disabilities.
Can work independently and as part of a team.
Has strong problem-solving and relationship-building skills.
Demonstrates professionalism, compassion, and a participant-centred approach.
Knows the NDIS and relevant service systems.
