East Grampians Health Service is seeking a part time Community Services Assessor in Community Nursing at the Ararat campus. You will assess eligibility for HACC PYP and work with younger people and Indigenous groups to support protected, independent living.

The role emphasises collaboration with a multi-disciplinary team, ongoing training, and building robust links with consumers, carers and local services. Immunisation clearance and employment checks apply.
