NSW Health’s CHW Long Term Ventilation Service in Westmead is seeking an experienced Allied Health qualified to coordinate LTV care for children and young people. The role spans inpatient, outpatient, community, and virtual care, focusing on discharge planning, service navigation, and multidisciplinary collaboration.

You will work across Monday to Friday with 24 hours per week (3 x 8-hour shifts), leading complex care coordination and developing services in a agile paediatric setting.
