About the Role

Working as part of the Community Allied Health team, you will provide social work services across three key programs:
Transition Care Program (TCP): Supporting patients transitioning from hospital into the community or longer-term care arrangements.
Rehabilitation in the Home (RITH): Providing psychosocial assessments,



care coordination and support to patients receiving rehabilitation in their homes.
Geriatric Evaluation and Management (GEM): Supporting older adults within an outpatient rehabilitation program to maximise independence and wellbeing.

You’ll work collaboratively with a multidisciplinary team to support patients and their families, facilitate discharge planning and coordinate appropriate community services.

About You
Tertiary qualification in Social Work
Eligibility for AASW membership
Previous experience in community health, rehabilitation, aged care or hospital social work
Robust assessment, discharge planning and case management skills
Ability to work independently and collaboratively within a multidisciplinary team
Available to commence 2nd November 2026
