As a Disability Support Worker with Vital Support Services, you will work collaboratively with participants and their support networks to provide protected, respectful and individualised care.Key responsibilitiesSupporting participants with activities of daily living, personal care and maintaining independenceProviding complex personal care according to the participant's individual care plan and your training and competencySupporting medication in accordance with the participant's care plan and organisational requirementsAssisting with mealtime support and other health-related support needsSupporting household and domestic activitiesProviding community access and assisting participants to attend appointments, social activities and other meaningful engagementsSupporting participants' emotional, social and general wellbeingObserving and reporting changes in a participant's health, behaviour, wellbeing or support needsWorking collaboratively with participants, families, support coordinators, nurses, allied health professionals and other members of the care teamFollowing incident reporting, risk management,
