NSW Health in the Far West Local Health District is seeking a Wellbeing and Health in-reach Nurse (WHIN) Coordinator CNS2 in a school setting.
You will coordinate health, wellbeing, and social needs with early intervention and referrals for school-aged children and young people.
You will lead complex care coordination with autonomy in a multi-agency context, supporting families and applying evidence-based practice to improve health outcomes.
