You will work as part of the Community Allied Health team, delivering social work across TCP, RITH and GEM programs, with accommodation and travel support provided.
The role focuses on discharge planning, psychosocial assessments, and coordinating community services for older adults and other patients, within a supportive multidisciplinary setting.
