Mackay Hospital and Health Service is seeking a Clinical Nurse Consultant (Connected Community Pathways) in Mackay QLD to lead a multidisciplinary team in delivering connected care for Aboriginal and Torres Strait Islander communities and others at risk of Type 2 Diabetes.
Join a dedicated unit focused on primary health care and prevention, improving access to care closer to home and reducing avoidable hospital presentations.
Permanent, full time role with flexible work arrangements.
