Catholic Healthcare Limited is seeking a Care Partner for an office-based role in Hunters Hill with travel across the Northern Sydney area.
The position is permanent full time, Monday to Friday, offering $***** per hour plus superannuation and kilometre reimbursement ($1.11 per km).
Responsibilities include relationship-centred care management, care planning, budgeting and billing accuracy to support clients living safely at home.
Strong collaboration with a multidisciplinary team is essential.
