Palm Lake Care seeks a Clinical Coordinator - Admission & Assessments.
You will lead foundational assessments, ensure accurate documentation, and develop individual care plans for residents.
You will coordinate AN-ACC reviews and funding through My Aged Care, balancing care needs with resident choice.
You will travel to other Palm Lake communities, mentor staff, and drive standards in documentation.
AHPRA-registered RNs with ANACC experience are encouraged to apply.
