Life Without Barriers is seeking a Care Partner in the Moreton Bay area to lead the delivery and continuous improvement of our in-home Aged Care services.
You will manage client referrals, support individuals as they commence services, and regularly review care plans to meet changing needs.
Travel across the region to visit clients in their homes and work with families to ensure high-quality, person-centred care.
