Life Without Barriers is seeking a Care Partner for Aged Care in the Moreton Bay area.
This permanent full‐time role involves leading the delivery and continuous improvement of in‐home aged care services, managing client referrals, care plans, and coordinating home visits to ensure high‐quality, person‐centred support.
You will collaborate with families, internal teams, and external partners, review processes, and contribute to program growth while travelling across the region to visit clients
