Palliative Care South East Ltd. invites a Nurse Practitioner to deliver autonomous, NP-led palliative care in clients' homes across the southeast Melbourne region.
You'll work with the Medical Officers, trainees, and the interdisciplinary team to deliver comprehensive assessment, diagnosis, treatment and care planning.
In this senior role, you'll build capacity in residential aged care and support GPs to embed a palliative approach, including participation in a rotating on-call weekend roster.
