About the Role
As the Clinical Care Coordinator, you will play a key leadership role in overseeing the delivery of clinical care across the home. You will support clinical governance, mentor nursing and care teams, and ensure residents receive safe, person-centred care in line with legislative and accreditation requirements.
Working alongside the Care Manager and Facility Manager, you will help drive continuous improvement initiatives and foster a positive, collaborative team culture.
Key Responsibilities
Coordinate and oversee the delivery of high-quality clinical care.
Support the management of AN-ACC processes, care planning, and clinical documentation.
Monitor clinical indicators and implement quality improvement strategies.
Assist in managing accreditation standards and compliance requirements.
Work collaboratively with residents, families, health professionals, and external stakeholders.
Assist with incident management,



infection prevention and clinical governance activities.
About You
Current AHPRA registration as a Registered Nurse.
Previous experience in aged care clinical leadership
Robust understanding of aged care legislation, accreditation standards, and clinical governance.
Excellent leadership, communication, and problem-solving skills.
Passion for resident-centred care and team development.
