The Albury Wodonga Health is seeking a Palliative Care Social Worker to join the Community Palliative Care Team in Victoria. You will conduct psychosocial assessments, provide counselling and coordinate care for patients, carers and families in community and outpatient settings.
You will work within a multidisciplinary team, delivering person-centred care and advocating for your clients, with access to career development and the support of regional health services.
