The Victorian Aboriginal Child and Community Agency (VACCA) is seeking a dedicated Key Worker to deliver intensive case management under the Targeted Care Packages (TCP) program. You will work with Aboriginal children, families and carers to reduce residential care and ensure culturally protected support plans are implemented.
With a small caseload, you will assess risk, develop, implement and review plans, and coordinate with stakeholders to achieve better outcomes for vulnerable children under 12.
