AnglicareSA is seeking an experienced Community Aged Care Service Partner to support older South Australians to live safely and independently in their own homes.
You'll coordinate personalised support plans, navigate the aged care system with customers and families, and work with providers to meet funding and service requirements. This role is based in Tonsley, full time and ongoing with a compassionate, person-centred approach.
