Together for Mental Wellbeing is seeking a dedicated support worker to establish trust with service users, help set goals, manage tenancies and move-ons, and work flexibly in homes and community settings.

You will conduct assessments, coordinate with other agencies, carry out risk assessments, attend reviews, and support individuals in daily tasks, personal care, and appointments, following infection control and Together procedures.
