Goulburn Valley Health invites applications for a part time Social Worker - Ambulatory Aged Care in Victoria.
You will assess patients, develop care plans, coordinate services and support families in the ambulatory aged care setting.
The role offers versatile schedule and a supportive multidisciplinary team within the Hume region, close to Greater Shepparton.
A qualification in social work and current registration are required, with experience in aged care preferred.
