Churches of Christ in Queensland on the Gold Coast is seeking a Care Manager/Partner for Home Care to help older Australians stay independent at home.

You will develop personalised care plans, coordinate services, manage risks and collaborate with families and clinicians to deliver quality outcomes. A current driver’s license and relevant qualifications are preferred for this permanent, full time role.
