You will collaborate with a Community Nursing Clinical Lead, registered nurses and healthcare assistants to deliver high-quality home-based care and reduce hospital admissions.

The role requires experience in community nursing, post graduate community qualification, and the ability to travel across the county with a valid UK driving licence and
