Based at the Salisbury Adult Community Mental Health Service and serving the northern Adelaide metro, the Transition of Care Clinician role focuses on moving consumers from tertiary to primary care in a person‑centred way.

You will collaborate with GPs and community providers, conduct mental state assessments and risk reviews, and help identify gaps in transition pathways while supporting team‑level quality initiatives.
