genU is seeking a caring and motivated Case Manager to join our Support At Home team in Geelong. You will work with clients, families and allied health providers to develop and review personalised care plans that focus on goals and funding requirements, delivering supports that are compliant and within budget.

You’ll coordinate services, monitor changes in client needs, and build robust relationships while navigating a busy, multi-disciplinary environment.
