About the Opportunity
Right at Home Greater Logan is looking for a motivated and compassionate Care Partner to join our growing team.
This is an opportunity to take ownership of a portfolio of clients, coordinate high-quality home care services, and make a genuine difference in the lives of older Australians who wish to remain living independently in their own homes.
We welcome applications from individuals with backgrounds in care coordination, case management, nursing, aged care and community services. Whether you're an experienced Care Partner or someone with relevant industry experience looking to take the next step in your career, we'd love to hear from you.
Salary & Remuneration The advertised salary range of $77,500 – $90,000 + superannuation reflects the varying qualifications, experience and capabilities candidates may bring to the role.
Applicants with entry-level qualifications, such as a Certificate III, and limited care coordination experience would generally be considered towards the lower end of the range. Candidates with higher qualifications (including EN or RN), extensive industry experience and demonstrated care management capabilities may be offered remuneration towards the upper end.
For an exceptional candidate with the right experience, qualifications and demonstrated capabilities, a salary above the advertised range may also be considered.
Performance-based bonuses may also be available in addition to base salary.
About Us
Right at Home Greater Logan is a locally owned and operated home care provider, delivering quality in-home care and support services throughout the Greater Logan and surrounding areas.
As part of the internationally recognised Right at Home network, we combine the resources and systems of an established organisation with the personal approach and accessibility of a locally operated business.
Our team is passionate about providing exceptional care to our clients while maintaining a supportive, collaborative and professional working environment.
We recognise that our Care Partners play a critical role in the success of our business, and we are committed to providing the resources, guidance and support they need to succeed.
About the Role
As a Care Partner, you will be responsible for coordinating and overseeing the delivery of services to an allocated portfolio of clients, primarily under the Australian Government's Support at Home program.
You will be a key point of contact for clients and their families, ensuring care arrangements remain appropriate, services are delivered effectively, and clients receive the support they need.
While you will take ownership of your client portfolio, you won't be expected to do everything alone. You'll be supported by our experienced clinical team, Operations Manager, scheduling team and dedicated Care Support Officer, allowing you to focus on client relationships, care coordination and quality outcomes.
Key Responsibilities
- Manage and coordinate an allocated portfolio of home care clients,
building strong relationships with clients, families and representatives.
- Undertake client assessments, reviews and care planning in accordance with individual needs, preferences and funding arrangements.
- Coordinate the delivery of services under the Support at Home program and other applicable funding arrangements.
- Monitor client budgets, service expenditure and funding utilisation to ensure services remain appropriate and financially sustainable.
- Work collaboratively with our scheduling team to ensure services are delivered in accordance with client needs and agreed care plans.
- Provide guidance and support to care workers, addressing service delivery concerns and escalating clinical matters where appropriate.
- Maintain accurate and timely client records, care plans and documentation using our electronic care management systems.
- Identify and respond to changes in client needs, risks, incidents and complaints.
- Ensure compliance with relevant aged care legislation, quality standards, organisational policies and procedures.
- Liaise with external healthcare professionals, service providers and other stakeholders to support coordinated care.
- Participate in a rotating after-hours on-call roster.
About You We are looking for someone who is organised, proactive and genuinely passionate about delivering quality care.
You will enjoy working with people, be comfortable managing competing priorities, and have the confidence to take responsibility for your own client portfolio while contributing positively to a wider team.
Importantly, we value attitude, initiative, communication skills and a willingness to learn, alongside formal qualifications and experience.
Essential Requirements
You will hold one of the following qualifications:
- Certificate III or IV in Aged Care, Disability, Individual Support or a related field; OR
- Enrolled Nurse (EN) with current AHPRA registration; OR
- Registered Nurse (RN) with current AHPRA registration.
Additionally, you will have:
- Experience in community-based aged care and/or disability services.
- An understanding of consumer-directed care, relevant funding models and quality standards.
- Sound awareness of clinical governance, risk management and continuous improvement principles.
- Current First Aid and CPR certification.
- National Police Check.
- Blue Card (Working with Children) or willingness to obtain.
- Current Australian Driver's Licence.
- Strong computer literacy and confidence using electronic client management systems.
- Excellent communication, organisational and problem-solving skills.
- Ability to manage multiple priorities and work effectively in a busy environment.
- A genuine team-focused, can-do attitude and commitment to service excellence.
- Willingness to participate in a rotating on-call roster.
Highly Regarded (But Not Essential)
- Previous experience in care coordination, case management or a Care Partner role.
- Experience working with Home Care Packages or the Support at Home program.
- Understanding of aged care funding, client budgets and service coordination.
- Nursing qualifications or experience in a clinical care environment.
- Experience using Visual Care or similar care management software.
- Demonstrated ability to independently manage a portfolio of clients.
Don't meet every preferred criterion? We still encourage you to apply. We recognise that valuable experience and transferable skills can come from a variety of professional backgrounds.
