About the Role

Supportive Journeys is seeking a reliable, compassionate and motivated Disability Support Worker to assist an NDIS participant with community access, daily living activities and personal organisation.

The role involves supporting the participant to become more independent, build confidence, maintain routines and engage in meaningful activities within the community.

We are looking for someone who is patient, understanding, organised and able to build a positive, professional relationship with the participant.

Key Responsibilities

- Community Access: Accompany the participant on outings, recreational activities, shopping trips and other community-based activities.
- Transport Assistance: Support the participant with travelling to appointments, activities and other destinations.
- Planning and Organisation: Help the participant plan their week, organise appointments, manage schedules and keep track of key tasks.
- Building Independence: Encourage the participant to develop life skills, make their own decisions and become more confident managing daily responsibilities.
- Social Participation: Support the participant in exploring interests, participating in enjoyable activities and developing positive social connections.
- Emotional Support: Provide encouragement, reassurance and a calm,



respectful environment when needed.
- Documentation: Maintain accurate shift notes and communicate relevant updates to management.

Requirements
- Previous experience in disability support or a related field is preferred.
- Solid communication and interpersonal skills.
- Patient, empathetic and respectful approach.
- Good organisational and time-management skills.
- Ability to encourage independence while respecting participant choice and control.
- Current NDIS Worker Screening Check.
- Valid driver's licence and access to a reliable, insured vehicle.
- First Aid and CPR certification preferred.
- Understanding of person-centred and trauma-informed support is highly desirable.

Pay: $33.50 – $34.50 per hour

Expected hours: 9.0 per week
