The role involves conducting assessments to identify needs of Indigenous clients and referring to appropriate home support services, with travel across metropolitan and regional locations.

You’ll collaborate with clients, families and multidisciplinary teams, perform IAT assessments, review support plans and work autonomously within a dedicated ACAS
