Palm Lake Care is seeking a Clinical Coordinator – Admission & Assessments (Registered Nurse - ANACC Coordinator) in Deception Bay. This full time role focuses on foundational assessments, documentation and care planning for residents, with ANACC reviews and funding managed through My Aged Care.

You will balance care needs with funding considerations, lead a multidisciplinary team, and travel between communities as required to ensure consistent standards and documentation excellence.
