Silverchain is seeking a Care Partner to support older Australians in the Central Coast region. You will conduct client assessments, develop personalised care plans and coordinate services to keep clients secure, independent and connected at home.

The role combines community-based client visits with the flexibility to work autonomously from home within a 6-month fixed-term contract. You’ll join a supportive team and work with clinical, scheduling and service delivery staff to deliver outcomes that
