Silverchain invites a TRANSPAC Care Partner to support older Australians in the North Sydney region, coordinating services and advocating for protected, independent lives at home.
In this 6-month fixed-term role, you’ll combine community client visits with the flexibility to work autonomously from home, working 75 hours per fortnight with hours 8:30am–5:00pm, Monday to Friday, and joining a supportive team with access to clinical, scheduling and service delivery colleagues.
