Silverchain is seeking a TRANSPAC Care Partner to support older Australians in the North Sydney region. You will assess needs, plan care, coordinate services and advocate for safe, independent living at home, combining community visits with the flexibility to work from home.

This 6-month fixed-term role offers full-time hours (75 per fortnight), autonomy to manage your caseload, and a collaborative team workplace with clinical, scheduling and service delivery support.
