Silverchain is seeking a Care Partner to support older Australians in the Southeast Sydney region, combining community-based home visits with the flexibility to work autonomously from home. You will assess client needs, develop personalised care plans and advocate for outcomes that help people stay safe, independent and connected in their own homes.

You will work with clinical, scheduling and service delivery teams in a supportive workplace, manage a caseload, and benefit from salary packaging
