Australian Unity Limited is seeking a Clinical Care Partner in Parkes, Forbes or Cowra to lead secure, coordinated care for customers with complex needs. This permanent full time role combines clinical leadership with escalation management within a multidisciplinary team and offers hybrid work.

You will support higher‑risk customers, identify clinical risks, undertake reviews, guide Care Partners, and contribute to governance and quality improvement.
