This role supports safe, appropriate and efficient medication use while contributing to effective patient flow and continuity of care across hospital and community settings.Working closely with multidisciplinary teams, you will deliver high-quality pharmacy services including medication reconciliation, medicines management, patient counselling and discharge support, while contributing to education, medication safety and service improvement initiatives.We are seeking a pharmacist to join our team in a part-time role, working 9:00am to 2:30pm weekdays, with a 30-minute lunch break.
This position offers flexibility and work-life balance while providing an opportunity to make a meaningful contribution to patient care.Key responsibilitiesDeliver clinical pharmacy services including medication reconciliation, medication supply, medicines information, patient counselling and discharge planning across sub-acute and inpatient servicesSupport safe and cost-effective medication use through formulary compliance, antimicrobial stewardship, restricted medicine management and medication safety activitiesCollaborate with medical, nursing, allied health and pharmacy teams to optimise patient care and ensure timely medication managementContribute to education, service delivery,
