Nepean Blue Mountains Local Health District (NBMLHD) is seeking a compassionate Clinical Aged Care Assessor to join our team at Lemongrove Campus.
You will assess care needs for older people across hospital and community settings, develop person-centred care plans, and facilitate access to aged care programs that promote independence and wellbeing.
As part of a multidisciplinary team, you will work across ACA sites, use My Aged Care knowledge, and support families and care providers.
